AEC project group

Sunderland Clinical Commissioning Group (SCCG) Urgent Care Board (UCB) commissioned a review of AEC across the whole unscheduled care system in Sunderland (October 2015). Although the review identified providers to have a good knowledge and understanding of AEC with a number of initiatives already in place, it recognised the need for a whole systems project group to support providers in working together to deliver whole system flow. Therefore a subgroup from the UCB has been commissioned to deliver the following expected outcomes:

  • City wide patient engagement strategy
  • Support AEC Pathways and Processes across the unscheduled care system
  • Develop a whole system AEC City wide action plan with complementary timescales between stakeholders
  • Engage with primary care in a standard way
  • Develop vity wide standard referral methods
  • Support interdependent work streams e.g. integrated teams and RAH
  • Support national and local urgent and emergency care strategies

The project group consists of stakeholders across the system from the following areas:

  • GPs
  • City Hospital Sunderland
  • Sunderland CCG
  • Recovery at Home
  • Urgent Care Centres
  • Community Integrated Teams
  • South Tyneside NHS Foundation Trust
  • North East Ambulance Service (NEAS)
  • Northern Doctors

The aims of the project group are to:

  • improve patient flow with the use of AEC – it’s a fundamental principle of modern emergency care
  • clinically change the way clinicians work and think
  • move from a traditional approach to admitting emergency patients to hospital thus providing a more holistic model of care

AEC is about getting patients the right care, at the right time, in the right place.  We have highly skilled community teams who can treat certain conditions just as well in the patient’s home as they can in hospital.  Please refer to the following video where key team members and sponsors discuss the importance of implementing AEC across the whole system in Sunderland.
