What have we achieved so far?
Technology Enabled Care Services (TECS)
TECS has been around in Sunderland since the 1980’s with its traditional Telecare service and extending this to Telehealth services in 2009. Overtime TECS has evolved to keep up with advances in technology and tested across the whole city involved all key providers of health and social care services.
Examples of this include medical devices which allow patients to monitor their health remotely and send readings via technology to their clinicians.
Wi-Fi in GP Practices and Digital Signage
Work has been carried out to greatly improve how we as CCG’s communicate to our patients. Across South Tyneside and Sunderland, Wi-Fi has been rolled out to more than half of GP Practices for public use in waiting rooms and digital signage has been installed to offer improved communication channels with key messages being displayed.
EMIS Web in all GP practices across Sunderland and South Tyneside
All GP Practices across South Tyneside and Sunderland use the same Clinical Information System – EMIS Web. Not only does this greatly improve the ease of information sharing, it has also established a collaborative best practice sharing approach meaning GP practices across the localities can learn from each other to improve patient services.
Global Digital Excellence
City Hospitals Sunderland Foundation Trust are leading the way in terms of their ‘digital maturity’. Over the years they have invested in a state of the art Patient Administration and Clinical System called MEDITECH. The Trust therefore considers that it is well in its way to being ‘paper-free at the point of care’, so much so that it was named a Centre for Global Digital Excellence by NHS England. This has secured the trust additional funding to build upon its existing MEDITECH solution, develop information sharing across the local health and care system and, importantly, delivers robust data security.
Sunderland Health Care Information Sharing
The All Together Better Programme based in Sunderland has been working to improve information flows between GP’s and Community Clinicians based in South Tyneside Foundation Trust. This includes community nurses, specialist GP’s and physiotherapy services. They are doing this to provide joined up and safer care, enabling informed decision making giving more time to spend on care. The Patient Leaflet giving more information is available here.
All GP Practices across South Tyneside and Sunderland offer GP Online Services to patients. This can be to book appointments, order a repeat prescription or to view your coded GP Record. This functionality is accessed via the EMIS Patient Access web portal or app:
This service is currently being promoted across the South Tyneside and Sunderland region as although all practices currently offer this, uptake is still quite low. So why not beat the queues and order your next prescription online?
Great North Care Record
NHS Healthcare professionals across the North East and North Cumbria have come together to improve the way patient information is shared to improve health outcomes for patients.
The Great North Care Record is a project led by GP practices, hospitals, community and mental health trusts with the purpose to make an agreed set of information from each organisation immediately available between health and care professionals using a secure, electronic system to help provide the best treatment.
For further information on what the Great North Care Record will mean to you as a patient or how to opt out, please visit the website – https://www.greatnorthcarerecord.org.uk/
Significant work has been completed to enable electronic prescribing functionality at GP Practices across the South Tyneside and Sunderland localities. This has resulted in all GP Practices offering this service to patients. Currently approximately 60% of repeat prescriptions are currently sent electronically meaning once a patient has requested their prescription; they can collect it straight from their chosen pharmacy without having to visit the GP Practice first. This enables greater choice and flexibility of how you access your medication. If you haven’t already nominated a pharmacy, please contact your GP Practice.
